Scenario Details

  • Diego is a 45-year-old unemployed man living at a group residence with other people with schizophrenia as well as people with a dual diagnosis of schizophrenia, and substance use disorder
  • Since the age of 24, Diego has had 12 hospital admissions for exacerbations of schizophrenia and has been on multiple different antipsychotics, including combinations. He has a history of receiving treatment with a first-generation oral antipsychotic but complained about feeling stiff and “like a zombie” and refused to continue on it
  • He is now on a second-generation antipsychotic and was doing reasonably well (getting along with his housemates, going to the neighborhood activity center, and engaged in treatment at the local CMHC) until about a month ago when he refused to leave the house and started refusing some of the meals
  • He has been responsible for taking his own medication, which was kept in his room
