Scenario Details

  • Dwight is a 43-year-old man who recently started seeing a new provider in a community mental health clinic after his previous provider retired
  • He had been seeing his previous provider for 3 years, and during this time has been treated with a second-generation oral antipsychotic, which he is tolerating well
  • On his first visit with his new provider, he reports that he spends most of his time at home, out of distrust of strangers. On mental status examination, he states that on occasion he hears a voice that tells him the future
  • He feels that the medication has helped him with his symptoms but also expresses frustration about the fact that he has not been able to move out of his parents’ house
