Scenario Details

  • David is a 26-year-old single male living with his parents. He was first hospitalized and diagnosed with schizophrenia at the age of 24
  • He responded well to initial treatment with a second-generation oral antipsychotic, but 5 months after discharge from the hospital, he felt that he no longer needed his medication and stopped taking it
  • Thirteen months later he experienced a psychotic relapse leading to a second hospitalization. He was treated again with a second-generation oral antipsychotic, but his response was only partial. He experienced residual paranoid delusions that his neighbors were watching his movements in and outside of his home and were reporting on his activities to the local police
  • He was started on an LAI, as his treatment team was concerned that he might not be taking his medicine consistently. Despite 5 months of LAI treatment, his paranoid ideation has not resolved
